Although information relating to the crash was sketchy as at the time of going to press, THISDAY learnt that the only two persons on-board the aircraft, the pilot and an engineer, survived the crash. IRS Airlines, which is one of the indigenous carriers that has operated for a long time, stopped scheduled services last October due to technical problems with its fleet. The airline, which operated three Fokker 100 aircraft, stopped operation when its aircraft developed hydraulic problem while landing at the Kaduna airport last year, prompting the Nigerian Civil Aviation Authority (NCAA) to ground its operations. The airline successfully carried a C check in one of its aircraft and was waiting for the return of the second aircraft that was taken for another C check for a long time, but it crashed in Niger Republic, few minutes before it could enter Nigeria's airspace. An official of the airline said the crash had done a death blow to the IRS, which had planned to resume operation on the arrival of the ill-fated aircraft, which would have doubled the number of its aircraft to meet the minimum requirement of NCAA. "I have not heard about the crash before now but I know that we are waiting for this aircraft to arrive today (yesterday) so that we could resume operation immediately. We already have one that had gone through C check; we are just waiting for this one. So if it has crashed it will be a great setback to us," lamented the IRS official. http://www.thisdaylive.com/articles/irs-aircraft-crashes-into-niger-republic/178282/ Back to Top Military jet crashes in Arizona Pilot had laceration to the nose after ejecting from crashing plane. PHOENIX, May 10 (UPI) --A pilot suffered minor injuries after his military jet crashed Friday afternoon in the desert south of Phoenix. The pilot, whose name wasn't reported, ejected from the Harrier jet just before it crashed at about 2:12 p.m. local time. Gila River Police found him two miles from the crash site. He was taken to Chandler Regional Hospital with a laceration to the nose. "Our Gila River Fire Department also has their hazmat team that's responded as well and gave initial responding police officers specific directions on what to do as far as the aircraft goes," said Detective Rob Hawkes. A spokesman for the U.S. Marine Corps Air Station Miramar said the jet was based out of Yuma, Ariz. The cause of the crash was under investigation. http://www.upi.com/Top_News/US/2014/05/10/Military-jet-crashes-in- Arizona/1451399734341/#ixzz31UtoVbgh Back to Top Safety Parachute Prevents Sydney Air Crash Disaster Pilot and three passengers escape with minor injuries after Cirrus light aircraft crash lands in front of Australia home. The Cirrus SR22 aircraft crash landed after the pilot deployed its safety parachute following engine failure at 4,000ft. A pilot and three passengers narrowly avoided disaster after a safety parachute allowed their light aircraft to crash-land in front of a house in Australia's Blue Mountains. All passengers on board escaped serious injuries after the Cirrus landed in front of a home in Lawson, west of Sydney, on Saturday. One passenger was taken to hospital suffering from neck pain. The Cirrus SR22 aircraft began spiralling towards the ground after suffering engine problems at an altitude of 4,000ft. The pilot activated the light aircraft's inbuilt parachute and managed to dodge houses and major power lines to drift to the ground. Local resident Robert Ross, who witnessed the plane landing, said it would have crashed into his house if it had not been for the parachute. "I looked up and the engine started to splutter," he told the Sydney Morning Herald. "He got it going again and then it went dead. "It then started to go into a spiral. I thought the pilot was going to eject but it all happened too quick. I started yelling out to my wife: 'There's a plane going to crash into the house'." Allan Bligh, president of the Sydney Flying Club, told the Herald that Cirrus light aircraft are fitted with a handle in the cockpit which releases a cover plate when pulled to deploy a parachute. "Then the aircraft is supposed to drift slowly to the ground but it doesn't always work to that effect - weather and other things can play havoc," he said. "There are a number of manufacturers who decline to use them and believe a controlled forced landing, which you are taught from your early days of flying, is a far better system than the deployment of a parachute." Cirrus says that its safety parachute has saved 87 lives as a result of pilots pulling the emergency lever in time to avert a disaster. Aircraft lands safely in Los Angeles. United Airlines flight from LAX to Hawaii turned back to Los Angeles Saturday because of smoke in the cockpit and cabin. No one was injured. A United Airlines flight arrived safely back at Los Angeles International Airport on Saturday after smoke was reported in the cockpit and cabin during a flight to Hawaii. Federal Aviation Administration spokesman Ian Gregor says United flight 1296 landed without incident at 11:38 a.m. Saturday, about three hours after it took off. United spokesman Charles Hobart says nobody was hurt. The Boeing 757 was headed for Kona International Airport with about 150 people on board when the smoke was reported by crew members. Fire Department engines and ambulances sent to the airport as a precaution were released after the safe landing. Hobart says passengers will be transferred to other flights and the aircraft will undergo a thorough inspection by maintenance crews. http://www.nydailynews.com/news/national/united-airlines-flight-bound-hawaii-turns-back-lax-smoke- detected-cabin-cockpit-article-1.1787532#ixzz31VFk1Vn3 Back to Top Back to Top FAA Safety Briefing Leveraging Certification and Standards to Avoid Monstrous Maintenance Mistakes | May/June 2014 The May/June 2014 issue of FAA Safety Briefing is all about Airworthiness Certification and Standards.
