The crew declared emergency and returned to Denver for a safe landing on runway 17R about 15 minutes after departure. The aircraft taxied to the apron. Passenger Peter M. Meyer reported the aircraft returned to Cologne due to a defect of the air conditioning system and posted a photo showing the cabin crew had donned their oxygen masks. Meyer): http://avherald.com/h?article=4b165653&opt=0 Back to Top Incident: Jet Airways B738 near Ahmedabad on Nov 21st 2017, cargo smoke indication A Jet Airways Boeing 737-800, registration VT-JGS performing flight 9W-2074 from Udaipur to Mumbai (India) with 111 passengers, was enroute at FL320 about 90nm east of Ahmedabad (India) when the crew received a cargo smoke indication and decided to divert to Ahmedabad for a safe landing about 20 minutes later. The airport reported the aircraft taxied to a remote stand, where all passengers disembarked. No trace of fire, heat or smoke was subsequently found. The aircraft remained on the ground for about 3 hours, then continued the journey and reached Mumbai with a delay of 3.5 hours. http://avherald.com/h?article=4b164c9e&opt=0 Back to Top Back to Top Incident: Meridiana B762 near Mauritius on Nov 13th 2017, engine shut down in flight A Meridiana Boeing 767-200, registration I-AIGH performing flight I-3811 from Mauritius (Mauritius) to Rome Fiumicino (Italy), was climbing out of Mauritius about 70nm north of Mauritius when the crew declared emergency reporting they had shut down one engine (CF6) and they wanted to return to Mauritius. During the return the aircraft deviated around weather and entered a hold due to weather (low visibility) at Mauritius. The crew reported they had everything under control, they did have a high temperature and fumes on the engine, the emergency was cancelled. The crew advised they would be landing at a higher than normal speed due to high landing weight. After landing the crew reported they saw smoke from the left main gear, they had landed at a higher than normal speed due to high landing weight. Emergency services cooled down the brakes. The passengers were taken to hotels and rebooked onto other flights the following day, a number were routed via Dubai (United Arab Emirates), a number via Milan Malpensa (Italy). The occurrence aircraft is still in Mauritius 10 days later awaiting an engine change. http://avherald.com/h?article=4b16531a&opt=0 Back to Top NTSB investigates two serious night-time runway incursion incidents at San Francisco Airport The NTSB has started an investigation into two serious night-time runway incursion incidents at San Francisco Airport, California. On 14 December 2016, a runway incursion occurred when SkyWest Airlines flight UA5086, a CRJ- 200, crossed the runway hold short line to runway 28L at the taxiway Juliet intersection while flight UA517 was on takeoff roll. Night visual meteorological conditions (VMC) prevailed at the time of the incident. Flight UA5086 was a CRJ-200ER, registered N986SW, operating on a flight to Ontario International Airport, California. Fight UA517 was a Boeing 737-924ER, registered N62849, operating on a flight to Washington-Dulles International Airport, DC (IAD/KIAD). Official data has not yet been released. ADS-B data from both flights as recorded by Flightradar24 suggests flight UA5086 had crossed the hold short line at 17:42:54. Six seconds later the aircraft was just short of the edge of runway 28L. Flight UA517 had initiated the takeoff roll at 17:42:36 passing Juliet at 17:43:06 at a speed of 160 knots. VX920 had crossed entered the runway as AA6081 was in final approach The second incident occurred on February 15, 2017, on runway 28L, when Virgin America flight VX920 was on a line up and wait clearance issued by the local air traffic controller and Compass Airlines flight AA6081 was cleared to land on the same runway. The air traffic controllers received an Airport Surface Detection System Model X (ASDE-X) alert and commanded the Compass flight to go-around, and the crew subsequently performed a go-around maneuver. Flight VX920 was an Airbus A320-214, registered N627VA, operating on a flight to Las Vegas- McCarran Airport, Nevada. Fight AA6081 was Embraer ERJ-175LR, registered N214NN, operating on a flight from Los Angeles International Airport, California. Night visual meteorological conditions (VMC) prevailed at the time of the incident (KSFO 160356Z 20019KT 10SM FEW100 BKN120 BKN180 16/04 A2999 RMK AO2 SLP155 T01560039). Official data has not yet been released. ADS-B data from both flights as recorded by Flightradar24 suggests flight VX920 was at the hold short line at 19:59:17 LT. At that time AA6081 was 3 NM from the threshold, at about 850 feet. At 19:59:32 VX920 had entered the runway and was in the process of lining up. At that time AA6081 was 2.4 NM from the threshold, at about 675 feet. ADS-B data suggest that AA6081 descended until at least 75 feet at 20:00:26. During the go around manoeuvre, the ERJ-175 crossed the paved surface short of the runway threshold at 100 feet and continued to climb, carrying out a safe landing 20:11. During the oceanic entry cruising at FL340 in the vicinity of waypoint ETIKI, the flight crew received intermittent amber 'A' messages on both PFDs, associated with AP TRIM IS RWD and AP TRIM IS LWD EICAS messages. Transfer of the autopilot from one side to the other did not resolve the problem and, as the flight crew decided to fly the aircraft manually, there was a rapid and significant roll of less than 20° to the right when the autopilot was disconnected. Extreme force was required to roll the aircraft back to the left, while turning to the right was normal. The flight crew executed the emergency checklist and requested a direct routing to Shannon Intl, Ireland (EINN). No emergency was declared, and aileron control improved as the aircraft descended. The aircraft subsequently landed on runway 06 with flaps 20 and ARFF standing by. During the post-flight inspection, the operator's maintenance noted that there was a significant amount of water dripping from the underside of the fuselage, aft of the wheel well in proximity of the lower beacon. The operator is suspecting that a water leak caused a freezing of the aileron trim mechanism. https://aviation-safety.net/wikibase/wiki.php?id=201545 Back to Top MC-21 wing box passes latest flight-load test Further stress testing of the Irkut MC-21 wing box has indicted that the structure will be able to withstand maximum flight loads. Irkut had stated earlier this year that it would need to reinforce the MC-21's wing box because, despite coming close to passing a destructive test, it had not achieved the strength threshold required. The airframer proceeded to commence flight testing of the initial aircraft in May. But the composite wing has undergone additional stress testing at the Moscow-based Central Aerohydrodynamic Institute. The institute says it has completed the latest phase of tests on the wing box in mid-November, in order to ensure the safety of the MC-21 flight-test fleet. It states that the wing box "withstood a load exceeding that specified by the test programme, without damage". Although it has not disclosed further details the institute says this has "confirmed" that the wing box has "sufficient strength" at maximum flight loads. The BIFT requirement was added when the country's CAA Part 61 was modified to align with International Civil Aviation Organization (ICAO) standards. As an interim measure, however, basic helicopter flight training without BIFT continues under transitional regulations that end on Aug. 31, 2018. "While there is a trend to more IFR operations, the sector continues to be dominated by small Robinson helicopters conducting VFR operations," officials said. "Robinson R22s and R44s make up approximately half of the helicopter fleet, and most lack the necessary instruments for BIFT." Also, most training aircraft are not currently equipped for instrument flight training. Mandatory instrument training experience would incur "significant cost and operational penalties." Additionally, the number of flight simulation training devices (FSTDs) is increasing, "which will expand the future opportunity for FSTD-based training, including BIFT." The proposal has the full support of the Australian Helicopter Industry Association and helicopter training operators. The helicopter industry has indicated that the issue of a license with a remark indicating non-compliance with ICAO standards would be acceptable. Speaking at a global ATFM symposium in Singapore ICAO secretary general Dr Fang Liu told the organisation's signatory states that they need to do this in order to safely and efficiently manage forecast growth, and optimise the social and economic benefits of increased international routes and flights. "To manage future growth we must become better at what we already do, and in the finite airspace we already control," Dr Liu emphasized during her opening remarks."For some States this will involve sovereignty concerns, as all partners involved in an ATFM framework must be willing to fully commit to and support a more open and collaborative international flight information environment." Dr Liu's points were echoed in the introduction she received from Kevin Shum, director-general of the Civil Aviation Authority of Singapore, who encouraged the attending governments "to build momentum and work towards introducing a global harmonised ATFM system." Shum also remarked that "the success of our industry depends on your spirit of openness and continued participation in the sharing of ideas, knowledge and experience." ATFM, which takes advantage of air transport Collaborative Decision Making (CDM) frameworks, optimises the existing capacities of the air traffic management system through the more precise coordination of take-offs and landings at both departure and destination airports, and through the dynamic routing of flights around constrained airspaces. "This advance and real-time coordination helps to avoid aircraft reaching their destinations and being placed into costly holding patterns, as well as permitting ATM professionals in general to be much more responsive when dealing with meteorological or other unexpected events," Dr Liu commented. Underscoring further that the speed and efficiency by which modern commercial aircraft can move passengers and cargo globally is the core value offering of international aviation, she stressed that these very qualities are being threatened today by how quickly air transport operations are expanding. Highlighting to her high-level audience that air traffic flights and passenger volumes globally tend to double every 15 years, Dr Liu placed emphasis on the fact that "in many mature air transport markets we no longer have the luxury of simply adding new airports and slots to accommodate further traffic growth. Competition for airspace is also accelerating, with new unmanned and commercial space-related services seeking to carve out their own niches for the expanding operations in those areas." Dr. Liu also reiterated ICAO's key points on why it is important that governments acknowledge how air transport traffic growth impacts modern societies and economies, and that the aviation system today moves more than 10 million passengers daily on over 100,000 flights. She noted how ICAO serves as a nexus for global cooperation toward optimising aviation's growth and socio-economic potential, and that it is of utmost importance that airlines, air navigation service providers, and airport operators collaborate more intensively and remain collectively diligent in managing airspace capacities and traffic demand. The event brought together more than 400 delegates from airlines, airports and technology providers to discuss how to unlock the value of big data in order to improve safety, security and profitability within aviation. "Big data is transforming our industry," said Charles de Gheldere, director, travel intelligence, IATA. "New aircrafts are basically data centers, generating terabytes of data; passengers are connected with their watches, with their phones and other devices -- there are sensors everywhere. Artificial intelligence is here, machine learning is here. "Data-driven organizations will take this industry to the next level. The sky is the limit." Representatives from airlines, airports and even data-driven companies outside of the aviation industry were on hand to discuss topics broken into four educational tracks - technology, safety and operations, passenger distribution and sales and airfreight data. The settlement was described in papers filed Tuesday in Manhattan federal court. Developers of the new World Trade Center buildings had once demanded $3.5 billion from aviation-related companies after hijacked planes destroyed three skyscrapers among five demolished buildings on Sept. 11, 2001. Lawyers said the agreement signed last week resulted from "extensive, arms-length negotiations" by lawyers "who worked diligently for months." The agreement also said the parties make no admissions or concessions with respect to liability for the attacks. "The court's approval of the settlement agreement will bring to a close this hard-fought 13-year litigation on terms agreeable to the parties," the lawyers said. Attorney Desmond T. Barry Jr., who submitted the papers to U.S. District Judge Alvin K. Hellerstein, declined to comment Wednesday. Developer Larry Silverstein and World Trade Center Properties have collected more than $5 billion from other defendants through lawsuits. The money has aided the reconstruction of buildings on the 16-acre lower Manhattan site. Earlier settlements included $135 million paid to a financial services firm that lost two-thirds of its employees. American Airlines spokesman Matt Miller said the company is pleased to have reached a settlement. "We will never forget that terrible day and its lasting impact including the tragic loss of 23 members of the American Airlines family," Miller said. United Airlines declined to comment. He said this while speaking with newsmen in Abuja He added that though the sector fared better in 2016, it still remained the riskiest skies to fly in. He said the Memorandum of Understanding (MoU) signed by the bank and the International Air Transport Association (IATA) was to seek ways to boost the African aviation sector to better serve the teeming populace. "First is, we are very concerned about the issue of air safety. "Africa, last year, did very well. It was one of the safest skies last year but it has not always been that way, so we are improving but that does not mean that we must actually rest on our oars on this. "So one of the things that we will be focusing on in the collaboration with IATA is how to improve investments from the bank and others on air navigation infrastructure, improving safety records and also making sure that we also improve the training facilities for the engineers, the air control traffic managers. "We are also looking at how to help to de-risk the market for accessing finance for infrastructure, in particular, air craft acquisition. "So these are the broad terms of the things that we are going to be working on but our focus is to make the aviation industry more competitive; deregulate the sector and make it more efficient. "Hopefully, also have financing that allows aircraft expansion to be able to cope with the rapidly increasing number of passengers that Africa needs and would have.'' The results of attendees' input in the roundup were released at the end of October in a "Runway Safety Pilot Best Practices" document that includes a wealth of practical safety tips and links to additional information on the topic. The surface operations category offers 19 recommendations to help pilots focus on taxiing, communication, and managing distractions. Six tips offer advice on how to use all available resources in the crew resource management section, while five additional pointers provide ideas for keeping aviation knowledge and skills sharp. "AOPA Fly-In participants at the Runway Safety Roundup not only walked away improving their own knowledge, but the results of their brainstorming can now help hundreds of thousands of other pilots," said AOPA Air Safety Institute Executive Director Richard McSpadden. "I encourage everyone to forward this list of best practices to the aviators in their community." The initiative is named after the legendary Australian female aviator, Nancy Bird Walton, and aims to boost the number of women pilots in the airline's ranks by encouraging young girls to make the necessary subject choices in high school. Captain Deborah Lawrie, a pioneer female pilot, says it's time for a women's museum. "Next year Qantas will hire more than 300 pilots across the group - I want us to up the ante with our female pilot intake," Mr Joyce will tell the Male Champions of Change Leaders forum in Sydney on Thursday. Qantas will commit to a 20 per cent intake of women in its 2018 cadet program, and double that number over the next decade to reach gender parity, at intake. Qantas CEO Alan Joyce wants the number of female pilots to grow. "We must work at the grassroots level to encourage girls and women into studies that can lead to a career in aviation," Mr Joyce will say. "We just need to reinforce the message that girls and women belong in technical jobs." The airline will target STEM (science, technology, engineering and mathematics) in schools, flying schools and cadet programs. "It is going to take a concerted effort but it's time for a moonshot vision for gender equality," Mr Joyce will say. The program will be named after pioneering pilot Nancy Bird Walton, pictured with her plane in 1934. In the past 12 months Qantas has almost doubled the number of female pilots in the pipeline. But "that figure's not as impressive when I tell you that the global average of female pilots is 3 per cent. Or when I tell you that only brings us to 18 per cent female pilots in the pipeline for our domestic and international airlines. "And it's not as impressive when I tell you that overall, across all of our airlines, including Jetstar and QantasLink, we are at just over 5 per cent female pilots - around 190 women compared to more than 3300 men." "Not when we know that our cabin crew have similar rosters and 68 per cent of our flight attendants are women. "Until we identify and accept the points of weakness in our systems and processes, female pilots will remain in the minority." Qantas pilots will keep going out into schools to serve as role models. "People like Georgina Sutton who was the first female chief pilot in Australia for Jetstar; Tegan Gray, the chief pilot for Express Freighters Australia; and Lisa Norman, who captained our new Qantas Dreamliner back from Seattle just recently." One-third of the Qantas board are women. Four of the nine of the leadership team are women. And 43 per cent of its overall workforce are women, and 50 per cent of its corporate graduates are women. Qantas has hit a target to have 35 per cent of its senior management team made up of women, but Mr Joyce says that isn't enough. The airline did "not apply enough rigour to processes for promotion into senior roles and, as a result, we missed a golden opportunity to surpass the target we had set ourselves". For the first time external recruits into its senior roles have been more than 50 per cent women. There had been big gains in technology roles. In addition, the meeting of the military investigator group is scheduled to take place on the afternoon of 22 May 2018. The aim of the seminar is to keep the European air safety investigation community abreast of current developments and evolving best practice in aircraft safety investigation. As in previous years, the seminar will include presentations on case studies, the European environment, challenges of modern air safety investigations and human factors in aircraft accidents and incidents. ESASI welcomes proposals for presentations to be given during the 2018 seminar, which should last a maximum of 20 minutes with a further 5 minutes for questions. Presentations should address issues relating to air safety investigations; particular areas of interest are: * challenges faced by air safety investigators; * the environment, and culture, that air safety investigators operate in; * practical experience of applying investigation techniques; * new techniques to aid the investigation; * topical case studies.
