Flight Safety Information - April 5, 2022 No.067 In This Issue : Incident: Nolinor B732 at Meadowbank on Mar 28th 2022, engine shut down in flight : Incident: Canada B789 at Toronto on Mar 26th 2022, slat did not retract : Incident: Indigo AT72 at Nagpur on Apr 4th 2022, smell of smoke on board : U.S. NTSB helps China download voice record of crashed Boeing jet : Call for Nominations For 2022 Laura Taber Barbour Air Safety Award : Position Available: CNS President and Head of Cargo, The Americas Incident: Nolinor B732 at Meadowbank on Mar 28th 2022, engine shut down in flight A Nolinor Aviation Boeing 737-200, registration C-GNLW performing flight N5-951 from Meadowbank,NU to Churchill,MB (Canada) with 64 people on board, was in the initial climb out of Meadowbank [CMB2] when the crew received an amber oil filter bypass indication for the left hand engine (JT8D). The crew worked the related checklists, shut the engine down, declared PAN PAN and diverted to Rankin Inlet,NU where the aircraft landed safely. The Canadian TSB reported maintenance determined the engine's magnetic chip plugs and the oil filter were contaminated with metal particles. The chip plugs were cleaned, the filter replaced according to the maintenance manual. After a ground run the aircraft continued to Montreal Mirabel,QC (Canada), where the engine (about 164 hours after overhaul) will be replaced. http://avherald.com/h?article=4f6f5210&opt=0 Incident: Canada B789 at Toronto on Mar 26th 2022, slat did not retract An Air Canada Boeing 787-9, registration C-FRTU performing flight AC-56 from Toronto,ON (Canada) to Dubai (United Arab Emirates) with 227 people on board, was climbing out of Toronto when the crew received indication that slat #5 had not retracted. The crew worked the related checklists, consulted with dispatch and maintenance, declared PAN PAN and decided to return to Toronto. The aircraft dumped fuel and landed safely back on Toronto's runway 33R about 75 minutes after departure. The Canadian TSB reported maintenance discovered the "slat skew detection outboard skew cable guide bolts on slat 5 were found to have been painted over and were not lock wired". A maintenance investigation is ongoing. http://avherald.com/h?article=4f6f56cd&opt=0 Incident: Indigo AT72 at Nagpur on Apr 4th 2022, smell of smoke on board An Indigo Avions de Transport Regional ATR-72-212A, registration VT-IYW performing flight 6E-7074 from Nagpur to Lucknow (India) with 54 people on board, was climbing out of Nagpur's runway 32 when the crew stopped the climb at about 8500 feet reporting the smell of smoke on board and decided to return to Nagpur. The aircraft landed safely on Nagpur's runway 32 about 35 minutes after departure. Emergency services did not find any trace of fire, heat or smoke. The flight was cancelled. The occurrence aircraft returned to service about 13 hours after landing back. http://avherald.com/h?article=4f6f58b1&opt=0 U.S. NTSB helps China download voice record of crashed Boeing jet WASHINGTON - The U.S. National Transportation Safety Board (NTSB) said Friday it was assisting Chinese investigators with the download of the cockpit voice recorder in Washington from an China Eastern Airlines 737-800 jet as it sent a team of investigators to China, David Shepardson reports for Reuters. 5 April 2022 - China and the United States have been in a bit of a political reproachment, but the Asian power has leaned on the U.S. NTSB in getting information in the deadly March crash. The fatal incident was mainland China's deadliest crash in nearly three decades, Reuters reports. The cockpit voice recorder, also called a "black box," may provide information to the cause of the crash that claimed the lives of 132 people. It is currently unknown if there was a mechanical malfunction, technological or human error. The NTSB tweeted on 1 April that its team "has departed for China to participate in CAAC's B-737 accident investigation. The Award is scheduled to be presented during Flight Safety Foundation’s annual International Air Safety Summit this fall. Presented annually since 1956, the Laura Taber Barbour Air Safety Award recognizes notable achievement in the field of civil or military aviation safety in method, design, invention, study, or other improvement. The Award's recipient is selected for a "significant individual or group effort contributing to improving aviation safety, with emphasis on original contributions," and a "significant individual or group effort performed above and beyond normal responsibilities." Mechanics, engineers, and others outside of top administrative or research positions should be especially considered. The contribution need not be recent, especially if the nominee has not received adequate recognition. Nominations that were not selected as past winners may be resubmitted for consideration in subsequent years. Please note that self-nominations will not be considered. The Award Committee, composed of leaders in the field of aviation, meets each year to conduct a final review of nominees and selection of the current year's recipient. About the Laura Taber Barbour Air Safety Foundation and Award The Laura Taber Barbour Air Safety Award's story dates back more than 75 years. On April 14, 1945, after visiting family in Pittsburgh, Laura Taber Barbour was aboard a Pennsylvania Central Airlines DC-3 when it crashed into the rugged terrain of Cheat Mountain near Morgantown, West Virginia. All passengers and crew were killed. In 1956 her husband, Dr. Clifford E. Barbour and son, Clifford E. Barbour, Jr., in close association with The Flight Safety Foundation, established the Laura Taber Barbour Air Safety Award in her honor. For the past 65 years, this distinguished award recognizing outstanding achievements in aviation safety worldwide has been presented at Flight Safety Foundation’s International Aviation Safety Seminar. In 2013, The Laura Taber Barbour Air Safety Foundation was formed as an independent non-profit charitable organization composed of members of the Award Board, the aviation community, and the Barbour family. The successful candidate will ensure the necessary separation of duties between the CNS and the IATA activities.
