The gear collapsed during the overrun. There are no injuries reported, the aircraft however received substantial damage. The airline confirmed a runway excursion that resulted in no injuries. Nigeria's NSIB reported the aircraft suffered a runway excursion at about 15:10L (14:10Z). The aircraft landed safely on JFK's runway 31L about 30 minutes later. The remainder of the flight was cancelled. The FAA reported: "AIRCRAFT ENCOUNTERD SEVERE TURBULENCE AND TWO FLIGHT ATTENDANTS WERE INJURED. DIVERTED TO KJFK." The aircraft landed safely on Charleston's runway 15, the crew requested an inspection of their wings by emergency services. A replacement Boeing 737-8 MAX registration N8881L reeached Manchester with a delay of about 5.5 hours. The aircraft landed on Atlanta's runway 26L about 40 minutes after departure. After landing the aircraft vacated the runway and stopped on the parallel taxiway, the crew advised no evacuation was necessary, they wanted emergency services to check the brakes due to overweight landing and would shut all engines down for emergency services to access the brakes. Emergency services reported no fire seen, about 30 minutes after touchdown the crew restarted the engines and taxied to the apron. The aircraft landed safely on Milan's runway 35R about 30 minutes later. All 11 people, a pilot and 10 passengers, aboard the Kenmore Air seaplane have been accounted for after the plane made an emergency landing and caught fire near Sucia Island in the San Juan Islands, the US Coast Guard told CNN. Four people were transported to hospitals with serious injuries, CNN affiliate KING reported citing a Kenmore Air spokesperson. One of the four injured passengers was transported in critical condition, San Juan County Sheriff Eric Peter said, according to KING. The victims' injuries range from "head injuries to broken bones and lacerations," Peter said. What prompted the plane to make an emergency landing is unknown. Voepass Flight 2283 crashed on August 9, 2024, in the town of Vinhedo, about 80 km (50 miles) northwest of Sao Paulo, capturing global attention after images of the plane's last moments in a stalled "flat spin," rotating more like a falling leaf than diving toward the ground, circulated on social media. The ATR 72-500 had departed from Cascavel, in southern Brazil, and was headed to Sao Paulo's Guarulhos international airport when it swirled out of control and plunged to the ground. The final report into the accident released by Brazil's ⁠Center for Investigation and Prevention of Aeronautical Accidents (Cenipa) found the aircraft encountered conditions favorable to severe icing and the plane's de-icing system experienced multiple failures, and the pilots switched it on and off several times amid icing warnings. A total of 19 factors contributed to the crash, Lieutenant Colonel Paulo Mendes Froes told reporters, including the crew's emotional state, external influences, adverse weather conditions and insufficient regulatory oversight. A preliminary report released in 2024 had identified signs of ice buildup on the aircraft but did not determine a definitive cause for the accident, as safety experts say air crashes are generally caused by a cocktail of factors. ATR 72 planes had previously been involved in other cases globally where pilots lost control after ice buildup. Cenipa issued a recommendation to the European Union Aviation Safety Agency (EASA), urging it to work with ATR to review the "degraded performance" procedure to help prevent crews from reaching similar limit conditions in future flights. ATR, the aircraft's manufacturer, is a joint venture between Europe's Airbus (AIR.PA), opens new tab and Italy's Leonardo (LDOF.MI), opens new tab. Cockpit recordings showed the captain saying that the de-icing system was malfunctioning, the report said, while the first officer stated at one point that he had forgotten to activate the ⁠system. Cenipa said the pilots may have been affected by phenomena known as "inattentional blindness" and "inattentional deafness," in which a high workload can cause operators to miss critical information. CULTURE OF INFORMALITY Beyond the flight itself, the report pointed to broader shortcomings at Voepass, describing what investigators called a "normalization of deviations" within the airline. Cenipa said previous failures involving the aircraft's de-icing system were not properly recorded in ⁠maintenance logs and cited evidence that some maintenance tasks were performed without appropriate supervision. "This stemmed from a culture of informality in which pilots and mechanics did not record aircraft-related issues in maintenance logs, with the aim of allowing the aircraft to continue operating the next day," Froes said. Cenipa also said the aircraft involved in the crash ⁠should not have been dispatched at the time of departure because of a fault in a windshield wiper system, though it added that did not contribute directly to the accident. The Boeing 777 landed safely, but this was far too close to the plot of Airplane! for comfort. All Three British Airways Pilots Fall Ill On Flight From India, One Incapacitated At 30,000 Feet British Airways has launched an investigation after all three pilots operating flight BA276 from Hyderabad (HYD) to London Heathrow (LHR) reportedly developed symptoms consistent with food poisoning. The roughly 11-hour flight was operated by a Boeing 777 with three pilots onboard, allowing crew members to rotate through rest periods during the longhaul journey. But there was apparently very little rest on this particular flight. One first officer became so violently ill while the aircraft was cruising at approximately 30,000 feet that he was considered incapacitated and placed on supplemental oxygen. The other two pilots also reportedly became sick, but remained capable of operating the aircraft and landing safely at Heathrow. Their symptoms worsened after arrival. British Airways insists that flight safety was not compromised. Technically, the flight did continue normally and land without incident. But when one of three pilots is incapacitated and the remaining two are also suffering from gastrointestinal illness during a 10.5-hour flight, it is difficult to argue that the safety margin was not reduced. Hotel Breakfast In Hyderabad Suspected The pilots had reportedly eaten breakfast together in the club lounge of their layover hotel before operating the flight. That meal is now suspected as the source of the illness, though no definitive cause has been publicly established. All three pilots filed Air Safety Reports and were asked to provide stool samples in an effort to identify what made them sick. More troublingly, representatives of the British Airline Pilots’ Association reportedly claim there had already been “dozens” of hygiene complaints involving the Hyderabad hotel. If that is accurate, British Airways has some explaining to do. A single isolated illness can happen anywhere. Foodborne pathogens are not confined to India, nor does becoming sick automatically prove a hotel was negligent. But repeated hygiene complaints followed by all three operating pilots becoming ill on the same flight merit careful scrutiny. British Airways said: “The welfare of our colleagues is a top priority and we are reviewing crew accommodation in this location following some isolated reports of sickness.” The airline is now reviewing its Hyderabad crew hotel arrangements. This Is Why Pilots Avoid Eating The Same Meal There is an obvious comparison here to the 1980 comedy Airplane!, in which the cockpit crew becomes incapacitated after eating contaminated fish. Surely this real incident was less dramatic, but the underlying risk is not fictional. Airlines commonly discourage, or in some cases prohibit, the captain and first officer from eating the same inflight meal. The purpose is straightforward: if one dish is contaminated, both or all pilots should not become ill at the same time. But that safeguard does little good if the entire augmented crew eats breakfast together before departure. I am not suggesting pilots must eat every meal separately during a layover. That would be impractical and rather miserable. But after an incident like this, British Airways may need to reconsider whether all operating pilots should eat identical food from the same buffet immediately before a longhaul flight. At minimum, maybe uncooked food should be banned (as I am currently learning…) and water should be sent form London? CONCLUSION All three British Airways pilots operating a flight from Hyderabad to London reportedly developed suspected food poisoning, with one first officer becoming incapacitated at 30,000 feet and requiring oxygen. The remaining pilots safely landed the Boeing 777, but British Airways is now reviewing its Hyderabad crew accommodation amid reports of previous hygiene complaints. This may sound like a scene from Airplane!, but it raises a very real safety concern. The incident occurred on April 11, when the captain of the plane flew the aircraft over the archipelago, which is located off the southern coast of Iceland. The captain, along with the first officer, has had their licenses stripped, with both being found negligent. The first officer has also subsequently lost his license for 12 months and has been on leave since the incident while the investigation was ongoing. According to Breaking Aviation News & Videos, the incident, which was captured and shared on social media by many, involved the Icelandair 757-200 operating a flyover of the Westman Islands of Iceland in April. This was done by the captain, who was operating his final flight for the Icelandic flag carrier and wanted to pass the aircraft over his hometown on the Westman Islands. The incident, which saw the aircraft fly below a safe altitude, resulted in an investigation by the Iceland Transport Authority and the Ministry of Infrastructure. Since being negligent, both pilots have the option of appealing the decision to the departments, but it is not clear whether this will proceed. Icelandair has also completed its own investigation of the flight, where it was confirmed by the airline's communication officer. The airline shared the following statement. "The results of internal investigations are confidential, but their goal is always to increase safety, and the report includes recommendations to that effect." The flight was operating between Frankfurt Airport (FRA) and Keflavik International Airport (KEF) on the aircraft that bears registration TF-ISR. There were passengers onboard the plane, which has raised immediate safety concerns about the act. While it can be common for farewell gestures to be undertaken by final flights for both captains and retiring aircraft themselves, these gestures must usually be pre-approved and conducted within operational limits. The move by the two pilots in this incident has drawn widespread attention due to the lack of authorization and the aircraft's maneuvers at a low altitude. However, the report sounds a cautionary note: despite drops in accident and fatal accident rates, total fatalities increased, highlighting the challenge of preventing extremely rare but catastrophic events as the world’s appetite for travel continues to soar. The data pertain to scheduled commercial air transport involving airplanes with maximum take-off mass (MTOM) over 5 700kg, and were published in ICAO's latest Safety Report today. The report tracks progress under ICAO's Global Aviation Safety Plan, which provides the framework for Member States, regional entities and industry stakeholders to develop strategies aimed at improving international civil aviation safety. In 2025, commercial airlines carried approximately 5 billion passengers on 38 million flights, both higher than in 2024. At the same time, the total number of accidents fell from 95 to 85. "These results demonstrate that aviation can and must continue to become safer even as more people fly," remarked ICAO Council President Toshiyuki Onuma. "They reflect ICAO’s leadership of decades of collaboration among States and industry to identify risks, learn from accidents, strengthen oversight, and improve safety worldwide." Fewer accidents despite more flights One of the clearest indicators of progress is the global accident rate, which measures how many accidents occur per one million flight departures. In 2025, the global accident rate improved from 2.6 to 2.2 accidents per million departures, a reduction of nearly 13 per cent. Put simply, airlines conducted more flights while experiencing fewer accidents overall. The strongest safety improvement in 2025 occurred in the area of fatal accidents, in which at least one person loses their life. The number of fatal accidents fell from 10 in 2024 to 4 in 2025, while the fatal accident rate declined by 60 per cent, from 0.27 to 0.11 fatal accidents per million departures. This is a significant achievement because ICAO's safety strategy focuses not only on reducing accidents but also on preventing the types of accidents most likely to result in loss of life. Zero fatalities in international aviation from accidents and acts of unlawful interference is one of the three aspirations in ICAO’s Strategic Plan. Why fatalities increased despite safety improvements Although there were fewer fatal accidents, the total number of fatalities increased from 296 in 2024 to 387 in 2025. Similarly, the fatality rate—the number of deaths per billion passengers carried—increased from 65 to 78. The reason is that a small number of accidents accounted for a large share of the lives lost. A single accident can involve hundreds of passengers. In 2025, one accident alone resulted in 260 fatalities, heavily influencing the annual total. This illustrates one of the central challenges facing modern aviation safety. The industry has become highly effective at preventing accidents, but eliminating the rare catastrophic events that can cause large numbers of fatalities remains a challenge and priority. Priorities ICAO concentrates its global safety efforts on accident types that have historically caused the greatest loss of life. These are known as Global High-Risk Categories of Occurrences as identified in the ICAO Global Aviation Safety Plan. They include: • Controlled Flight into Terrain (CFIT): when a functioning aircraft unintentionally flies into terrain, a mountain, water, or another obstacle, often because the crew is unaware of the danger. • Loss of Control in Flight (LOC-I): when pilots can no longer maintain control of the aircraft during flight. These events are rare but often severe. • Mid-Air Collision (MAC): when aircraft collide or nearly collide while airborne. • Runway Excursion (RE): when an aircraft overruns the end of a runway or veers off its side during takeoff or landing. • Runway Incursion (RI): when an aircraft, vehicle, or person is incorrectly present on an active runway, creating the potential for collision. Together, these five categories accounted for more than half of all fatal accidents in 2025. The category requiring the greatest ongoing attention is Loss of Control in Flight, which accounted for nearly one-third of fatal accidents among ICAO's high-risk categories. Because these accidents often develop rapidly and leave little opportunity for recovery, preventing them requires continual investment in pilot training, aircraft systems, safety management, and human-performance initiatives. CFIT remained a significant concern in 2025 because, although only one CFIT accident was recorded, it accounted for more than 10 per cent of all fatalities worldwide. This reinforces a longstanding pattern in aviation safety: CFIT accidents are relatively infrequent but often have catastrophic consequences when they occur. The data suggest that continued attention to terrain-awareness technology, flight crew situational awareness, and operational safeguards remains essential to further reducing fatalities. The four MAC-related accidents in 2025 accounted for nearly 15 per cent of all fatalities worldwide—making them one of the most lethal high-risk categories when they occur. The data reinforce the importance of collision-avoidance technologies, air traffic management systems, and maintaining safe separation between aircraft in increasingly busy and complex airspace. As air traffic continues to grow and new entrants such as drones and advanced air mobility vehicles are integrated into the airspace system, managing collision risk will remain a critical global safety priority. On a positive note, the report also highlights progress in runway safety. No runway incursion accidents involving commercial air transport were recorded in 2025. Preventing these events is a major focus of ICAO's Global Runway Safety Action Plan. The absence of such accidents in 2025 suggests that investments in airport procedures, technology, communications, and pilot awareness are producing positive results. The aviation community must continue to build on this progress, as seven accidents related to runway excursions did occur that year, one of which resulted in fatal injuries. Strong safety systems are essential Beyond accident statistics, ICAO also assesses how effectively national aviation authorities oversee safety. This is measured through ICAO's Universal Safety Oversight Audit Programme, which evaluates whether States have the laws, regulations, inspectors, technical expertise, and oversight systems and State Safety Programmes needed to ensure safe aviation operations. The report shows that global safety oversight performance continued to improve in 2025, with the worldwide average score reaching 698.8 per cent. The number of States meeting ICAO's target level of safety oversight also increased. However, challenges remain. The greatest weaknesses identified globally relate to: • Resolving identified safety issues; • Maintaining sufficient numbers of qualified technical personnel; • Conducting effective ongoing safety surveillance and inspections. This year’s jump in fuel costs has eaten into airline earnings. Carriers have raised fares, and executives this month said they expect higher prices to stick this year. Southwest Airlines hired a ship this spring to send jet fuel from Texas to California, where prices are much higher and concerns had grown about supply, Chief Financial Officer Tom Doxey told CNBC. It was a first for the Dallas airline. “It brought like a week’s supply to the West Coast at a time when when supply was most constricted ... when it was most at risk,” Doxey said. The ship, which left from Houston and went through the Panama Canal, arrived May 28 in Los Angeles and had about 12.6 million gallons aboard, Southwest said. For context, Southwest used 564 million gallons of jet fuel in the last quarter. The West Coast is much more reliant on imports than other parts of the country. Jet fuel prices spiked and have been volatile since the U.S. and Israel struck Iran in February. Southwest said Thursday that its fuel expenses were up nearly $900 million in the second quarter from last year. For the shipment to California, the airline said it used a waiver of the Jones Act, a law from 1920 that requires shipments between U.S. ports to be carried on a U.S. ship. President Donald Trump waived that requirement in March as fuel prices were soaring in the weeks following the start of the Iran war and subsequent shipping snarls erupted in the Strait of Hormuz, a key channel. Worries about supplies intensified as countries restricted exports this year, fearful of running low on fuel. Those concerns have since eased, a Southwest spokesman said. Jet fuel is airlines’ biggest expense after labor. Prices eased in late spring and early summer but rose again as tensions reignited with Iran this month. Last week, United Airlines , which flies more internationally than any other U.S. carrier, said it is using the latest available fuel prices for its quarterly estimates because prices have been so volatile. In its July 15 report, it said jet fuel increased $575 million, or a $1.12 hit to adjusted earnings per share, for the third quarter alone. U.S. airlines have abandoned fuel hedges, which help them lock in costs through futures contracts, over the past decade or so as the U.S. was awash in supply, keeping a lid on prices. This time around, carriers have scaled back their capacity growth plans, which is also helping boost fares.
